FREEHOLD TOWNSHIP — Registration packets for the 2009-10 Freehold Township School District full-day kindergarten program are now available in the elementary schools during regular school hours.
The registration packets may be picked up at any of the five schools — Applegate, Catena, Donovan, Errickson or West Freehold — or they can be downloaded off the district’s Internet Web site at www.freeholdtwp. k12.nj.us.
To be eligible for registration, children must be 5 years old on or before Oct. 1, 2009, documented by a birth certificate, and have two proofs of residency. Students must have all immunizations including four doses of DTP, three doses of polio vaccine, MMR, three doses of hepatitis B vaccine and one dose of varicella vaccine or a history of chicken pox. A complete physical examination performed after Sept. 1, 2008, must also be reported on Form N107 and signed by the physician. All forms and requirements are included in the registration packet.
Registration will take place at the schools on March 11 from 1 to 3 p.m. for students whose last names begin with A-M and on March 12 from 1 to 3 p.m. for students whose last names begin with N-Z.
