JACKSON — A resident of Whiting has been charged with death by auto two months after a motor vehicle accident in Jackson claimed the life of a Toms River woman.
The May 30 two-vehicle crash that resulted in the death of Colleen Guierro, 39, of Toms River, occurred on South Hope Chapel Road.
According to information provided by Jackson police Capt. David Newman, Police Officer Trevor Crowley completed his investigation of the crash and charged Jean Paul Forde, 31, of Whiting, with death by auto (vehicular homicide) and assault by auto.
Forde was also charged with the traffic offenses of driving while intoxicated, reckless driving and failure to maintain lane.
Police allege Forde was driving a 1994 Honda Accord while under the influence of alcohol and at a high rate of speed when he lost control of his vehicle and crashed into a 2007 Chevrolet Cobalt driven by Guierro.
Newman reported that Guierro was pronounced dead at the scene of the accident. Her daughter, who is a minor, was a passenger in the vehicle and survived her injuries.
Forde was arrested at Jackson police headquarters on July 29. His bail was set at $100,000 and he was initially lodged in the Ocean County Jail, Toms River.
