Healthcare needs new approach

As the healthcare delivery landscape continues to evolve, effective collaboration among providers will be critical.
Resources and reimbursement levels continue to shrink, and patient needs continue to rise, demanding a new approach.
Bringing together the clinical and operational expertise of providers across the continuum of care will result in more seamless care transitions, lower cost and most importantly, better quality of care. In response to this evolution in care, Visiting Nurse Association of Central Jersey and Springpoint Senior Living recently launched a collaboration that combines 193 years of collective health care experience.
Springpoint Senior Living is a leading provider of senior housing and services, including continuing care retirement communities, assisted living facilities and affordable housing.
VNA of Central Jersey, a member of VNA Health Group, is the state’s largest non-profit provider of home health, hospice and community-based care.
Our new alliance will deliver several tangible benefits to those we serve. VNACJ’s patient and family-centered end-of-life program will augment Springpoint’s excellence in skilled care services to afford residents a full continuum of care provided by two premier providers. Springpoint residents in need of more complex care, often necessary to treat complicated chronic conditions such as, diabetes, heart failure, cancer, transplants, and acute infections, will be able to receive specialized care from the comfort of their own home.
Some of these specialized care services include infusion therapy, pain management and advanced wound care. In addition, on-going health education and prevention programs, provided by expert healthcare professionals, will be available to all residents and surrounding community members.
Springpoint residents and physicians will also benefit through VNACJ’s investment in cutting edge, innovative technologies. Physicians will have 24-hour access to the latest patient information through the VNACJ electronic health record and a physician web portal.
Telehealth in-home monitoring will be available for patients with acute or chronic illness such as congestive heart failure for 24-hour monitoring and targeted nursing intervention that has shown significant outcomes that reduce unnecessary trips to the emergency department and hospital admissions.
Healthcare is changing. Those who deliver healthcare and housing-related services must find new and innovative ways to work together toward a common goal: keeping people we serve healthier longer, and helping them remain in their homes when they become ill and need care.
