Reformed Church Home, a nonprofit skilled-nursing, rehabilitation and assisted-living community in Old Bridge, recently was named a winner in the 2015 McKnight’s Excellence in Technology Awards competition.

Implementation of the EarlySense continuous monitoring system earned the home a certificate of merit in the Dignity Through Technology category, which recognizes long-term care communities that have harnessed technology to improve the level of dignity for their residents. Continuous, contact free monitoring of rehabilitation residents with the EarlySense System enables the clinical team at Reformed Church Home to provide care at the bedside before a potential adverse event occurs.

Such proactive care is a top priority for the facility, as it strives to reduce hospital re-admission rates for its residents, a key challenge for all rehabilitation centers.
