Home care professionals help patients remain at home 

November is National Home Care and Hospice Month, a recognition of the thousands of health care professionals and related staff who work tirelessly every day to provide patients and families vital health and nursing care in the best place imaginable — their own homes.
The Home Care & Hospice Association of New Jersey represents more than 36,600 visiting nurses, home health aides, hospice nurses, therapists, and social workers and other affiliated agencies and employees who serve more than 98,000 Medicare beneficiaries, and in excess of 150,000 total patients.
A growing number of New Jerseyans are turning to our member agencies to receive long-term care in their homes, or end-of-life care in a compassionate and nurturing hospice setting. Our commitment is to not only ensure that the needs of patients and families are met, but that our members operate and perform at the highest level of effectiveness.
Our professionals are trained to spot warning signs before a trip to the hospital is the only option, and work directly with patients and families to ensure a greater sense of wellness. Many of these patients are elderly and managing chronic illnesses. Our services help keep threats at bay, not only to their well-being, but to the independence they have worked so hard to maintain. With new technologies alongside skilled nursing know-how, our members are helping patients live stronger, more meaningful lives in the place where they feel safest and most comfortable.
