The CentraState Healthcare Foun-dation has received a $70,000 grant from the Robert Wood Johnson Foundation and a $25,000 grant from Verizon to continue its Telecare Connection disease management project for an additional 12 months. Telecare Connection Disease Management is an Internet-based program designed to help patients, family members, and professional caregivers manage the care of congestive heart failure and diabetes from remote locations.
The program was launched in 2004 through the Monmouth County Chronic Care Consortium, a public-private partnership composed of CentraState Health-care System, the Monmouth County Office on Aging, and the Visiting Nurse Association of Central Jersey (VNACJ).
The “Share and Support” service offers daily, Web-based health education along with a communications network. Participants use a computer to access the service, which provides a medication inventory with reminders, remote monitoring of vital signs, nutrition plans and more. Clients also can view health education programs, participate in fitness activities, view live Webcasts of local events, and send and receive e-mail, among other functions. Nurses and care managers can conduct individual “tele-visits,” monitor vital signs, and provide education, support and training.
United Tae Kwon Do Academy’s Manalapan location joined forces with Master Bryan Klein’s four other New Jersey schools to raise more than $50,000 in funds for the Cystic Fibrosis Founda-tion. More than 500 students gathered inside Brookdale Community College’s Robert J. Collins Arena and performed various punches, kicks, blocks, katas (forms), weapons and board-breaking and self-defense routines for family, friends, and members within the community during a two-hour demonstration.
This year, United Tae Kwon Do Academy raised money for cystic fibrosis because this disease affects one of their students, Michael Taylor, and his sister. Cystic fibrosis is a genetic disorder that causes the body to produce thick mucous that clogs the lungs and other ducts throughout the body. Children like Michael, 9, a blue belt at United Tae Kwon Do Academy in Matawan, undergo daily lung treatments just to stay alive.
