MONROE – The Meadowview Care Center has been closed by order of the state, effective immediately.
“The conditions at Meadowview Care Center posed a serious threat to the health and safety of both the facility residents and the residents of the Township of Monroe. We appealed to the bureau who sought to have the center’s operating license revoked,” Mayor Stephen Dalina said in a prepared statement. “I am relieved and happy to report that the facility is now closed.”
The State of New Jersey Department of Community Affairs, Division of Codes and Standards, Bureau of Rooming and Boarding House Standards, sent a notice of Cease and Desist with orders to vacate the building at 319 Spotswood Gravel Hill Road.
The notice was issued to Sapphire Assisted Living and Guru Venkataraman on April 13 for failure to provide sufficient staffing to provide a resident supervisor 24 hours per day by an employee who is awake on the premises; not having an administrator available in the facility; failure to provide health maintenance monitoring service under the direction of a registered professional nurse; failure to maintain nurses’ monitoring notes that should be written on an ongoing basis that reflect the current health status of the resident; failure to provide proof of training for appropriate staff persons responsible for supervision of residents’ self-administration of medications; and failure to maintain a daily record of the type and amount of medication taken by the resident and the time at which such medication is taken.
In addition, on May 17, Middlesex County Prosecutor Yolanda Ciccone and Chief Michael J. Biennas of the Monroe Township Police Department released information that Deymis Jimenez, 32, of Monroe, was charged with murder and unlawful possession of a weapon following an investigation by Detective Joseph Gentile of the Monroe Township Police Department and Detective Mark Morris of the Middlesex County Prosecutor’s Office.
On May 17 at approximately 12:36 a.m., authorities found the deceased adult female in a common area of the building. The victim was pronounced dead at the scene.
The investigation determined Jimenez had assaulted the victim in a common area in the building.
The Township of Monroe police department, legal counsel and administration have been advising the NJDCA Bureau of Rooming and Boarding House Standards of continuing issues regarding the operation and management of the facility, Dalina said.
“I would particularly like to commend MTPD Sgt. Ronald Breuer for his ongoing diligence and commitment to ensuring the health and safety of all involved,” Dalina said in his statement.
Meadowview Care Center and Sapphire Assisted Living could not be reached by press time.
