this form and photo via EMAIL: Send your photo as a .TIF or .JPG
file. The picture may be any size, but a black and white photo (200dpi)
is preferred. Please include bride’s full name and the wedding date
in the email. Email your photo to [email protected],
then complete and submit the form below.

How to send your form and photo via U.S. Mail: Any size photo is
acceptable; a black and white glossy photo will reproduce best.
If you would like the picture returned, please include a stamped,
self-addressed envelope. PRINT, complete and mail the form AND photo
to the Packet office as soon as possible:
