Mrs. Formaniak, 76, of Jamesburg, died July 25, 2009, in Raritan Bay Medical Center, Old Bridge division. She was predeceased by her husband, Thomas Formaniak Sr., and by a sister, Kathleen Rennon. Surviving are a daughter, Diane Formaniak Zielinski, and her husband, Robert, of Naples, Fla.; a son, Thomas Formaniak Jr., and his wife, Laurie, of St. Petersburg, Fla.; a sister, Ann Rennon Boege, and her husband, William, of Colts Neck; a brother, J. Paul Rennon, and his wife, Shirley, of Milltown; three grandsons; a great-granddaughter; and several nieces and nephews. Arrangements were under the direction of Lester Memorial Home, Jamesburg. A private graveside service was held at Holy Cross Burial Park, South Brunswick.
